Data Protection Agreement

In accordance with the Data Privacy Act of 2012 (Republic Act No. 10173) and its Implementing Rules and Regulations, I consent to Eagles PharmaHealth (EPH) collecting, recording, storing, using and processing my personal and sensitive personal information, including health information contained in prescriptions, for the following purposes:
- processing, verifying and delivering my orders;
- pharmacist review of prescriptions and dispensing records required by law;
- contacting me about my orders through SMS, email, push notifications or calls;
- payment processing and fraud prevention;
- improving EPH services.
EPH shall protect my data with reasonable organisational, physical and technical security measures, share it only with personnel and service providers who need it for these purposes (such as riders, payment and SMS providers), and retain it only as long as necessary or as required by law.
I understand that I may withdraw this consent, and exercise my rights to be informed, to access, to object, to erasure or blocking, to rectification and to data portability, by contacting EPH's Data Protection Officer. Withdrawal may prevent EPH from processing my orders.
